Our Occupational therapists (OT) are highly experienced with Discharge Planning for both the Pediatric and Adult population.
Our experience working in a hospital setting as well as the community provides us a unique skill set which allows us to work collaboratively and efficiently with the hospital-based OT to encourage a safe transition from hospital to home. Our dual perspective as both hospital-based and community-based OTs helps us to better understand what may be required to meet each individuals’ unique needs.
We have a good working relationship with medical vendors throughout Manitoba and recognize the value of working together with the client/caregiver and medical vendors to determine specific equipment available to allow a client maximal independence in the home environment and to ensure it is in place prior to hospital discharge.
