Our Occupational Therapists (OTs) are highly experienced and able to complete a variety of Housing Assessments with the client’s consent. This may include:
- A Housing Assessment to guide discharge planning. A detailed report, including pictures, is shared with the treating hospital-based OT to:
- Guide a client’s hospital therapy program specific to what they will be required to do in the home environment.
- Recommend equipment options for safe, independent ADL.
- Recommend potential modifications to the home environment for safe, independent ADL.
- Identify if the current residence is an accessible and safe option for discharge planning.
- A Functional Assessment is completed with the client in their home. The information is used to provide recommendations to encourage a client to access and remain in their home safely and independently.This would include a comprehensive assessment guided by the areas of concern identified by the client and may include recommendations such as:
- Possible modifications to the home which would be completed by a medical vendor or a company specialized in home modifications (e. g. installation of thresholds, ramp(s), wheelchair lifts, chair lifts, etc.)
- Equipment options to allow a client to safely and independently complete activities of daily living (ADL) which may include: transfers from different surfaces, bathe/shower, dress, complete meal preparation, etc.
